If a student needs to take prescribed or over-the-counter medication, including oral or topical treatments, eye drops, or ear drops, while at school, the parent or guardian must provide written permission. This permission must be accompanied by a written request from a licensed health professional, as stated in School Board Policy 3416 - Medication at School. All the forms provided below require completion by both the student's parents or guardians and the Licensed Health Professional (LHP) before being returned to the school.
If your child requires a special dietary accommodation for medical reasons, please submit the Special Dietary Accommodations form. The form must be signed by the parent/guardian and a licensed healthcare provider.
If your child requires a medical treatment while at school, please submit the relevant treatment authorization form. The form must be signed by the parent/guardian and a licensed healthcare provider.
