If a student needs to take prescribed or over-the-counter medication, including oral or topical treatments, eye drops, or ear drops, while at school, the parent or guardian must provide written permission. This permission must be accompanied by a written request from a licensed health professional, as stated in School Board Policy 3416 - Medication at School. All the forms provided below require completion by both the student's parents or guardians and the Licensed Health Professional (LHP) before being returned to the school.
All the following forms, excluding the Authorization for Exchange of Information, must be filled out by both the student's parents or guardians and the Licensed Health Professional (LHP) prior to submitting them back to the school.
